- Enter your name, phone number, and email address in case we have any questions for you.
- We’d like to know how you heard about the Paint It Forward program, so please be specific on the next question.
- Enter the nominee’s name, phone number and address (or town, if you don’t know the whole address).
- The last question is the most important: their story! What made you think of this person or family? How will some interior painting help them out?
- The confirmation button for the last question says OK, but be sure you also hit the SUBMIT button to send us your nomination!
